How the hell did the shooter get to such a prime position?
Why didn't the Secret Service take him out after numerous people pointed him out.
Why didn't the closest Secret Service not react more quickly?
Why did they let him put his shoes on and then let him pose for an already iconic photo? His head clearly available for another shot.

Wut? He was shot in the ear with the 2nd of 4 or 5 rounds, before he had time to duck behind his podium the Secret Service had returned fire killing the gunman.

There isn't more to this than an insufficient (distance) security perimeter and local cops that were slow to react to members of the publics concerns.
